Admission Guide

Admission reservations

If you are to be admitted, please go to the Patient Support Center (2nd floor) to complete the necessary procedures.
(8:30 am - 5:15 pm, excluding Saturdays, Sundays, national holidays, December 29 - January 3)
A nurse interview will also be conducted if required.

* The video tour of the hospital's ward rooms is available in Japanese only.

Notification of admission date

Patients who have a scheduled admission date

We will contact you by phone or SMS message on the day before the admission date.
* If the admission date is a Monday, we will contact you on the preceding Friday. If you do not receive notification from us by 4:00 pm, please contact the admissions section.

Patients who do not have a scheduled admission date

We will contact you by phone on the business day before the day you can be admitted, as soon as a bed becomes available.

Admission procedure

Please come to the Patient Support Center (2nd floor) at the designated time (usually around 10:00 am) that we will inform you of when we contact you on the day before admission .
* The admission time may be changed if the hospital is busy.

Items Required for Admission

  • Health Insurance Card
  • Benefit certificates
    (Aged pensioner certificate, disability pension certificate, high-cost medical care benefit certificate, etc.)
  • Patient Registration Card
  • Admission application form (and pledge) (See page 25 of the "Admission Guide")
  • Discharge certificates (if you have been admitted to any hospital including this hospital within the past 3 months)
  • Medicine Pocketbook
  • * If your health insurance card or benefit certificate changes or expires during your hospitalization, please promptly present it to the Patient Support Center (2nd floor).
